FLOATING SUPPORT

ONLINE referral AND ASSESSMENT FORM

With effect from 1st April 2026 Tabono Homes will be providing a floating support service to complex women who have been newly housed in permanent accommodation or those at risk of tenancy failure with the aim of preventing eviction and future homelessness.

Our Floating ‘Prevention, Crisis and Sustainment Outreach’ Housing Support Service will be built on and around the structures of assertive outreach, and operate Monday-Friday 9am-5pm. Staff will have the flexibility to work outside of these hours where there is a need.

Our support workers will meet clients in their homes (following a risk assessment), or in locations where they feel comfortable and safe.

ONCE SUBMITTED TABONO HOMES WILL CONTACT THE PERSON SUBJECT OF THE REFERRAL WITHIN THREE WORKING DAYS TO ARRANGE A FACE TO FACE APPOINTMENT TO DISCUSS THE REFERRAL FURTHER. PLEASE NOTE ADDITIONAL CHECKS WITH OTHER AGENCIES ENGAGING WITH THE PERSON MAY BE CARRIED OUT BEFORE FLOATING SUPPORT IS OFFERED THEREFORE A DECISION MAY NOT BE IMMEDIATE.
