ONLINE referral AND ASSESSMENT FORM

Tabono Homes provide supported accommodation to vulnerable persons with low, medium or high risk needs (as determined by us). We accommodate persons of all genders over 18 years of age in same sex accommodation. We will consider housing:

• persons with multiple complex needs

• persons who are homeless or whom have a history of rough sleeping, sofa surfing or living in inappropriate accommodation

• persons at risk of exploitation (any form), those with mental health needs

• persons with learning disabilities and/or physical disabilities

• persons affected by substance misuse, victims of trafficking (if not wanting to access the National Referral Mechanism)

• persons who sex work

• persons who have experienced the Criminal Justice System including those being released from prison

• persons leaving refuges who are not suitable for unsupported accommodation

This form should be completed by an appropriate professional in as much detail as possible and ideally by someone who has sufficient background knowledge of the person being referred.

Providing us with as much detail as possible will enable us to process the application quicker.

ONCE SUBMITTED TABONO HOMES WILL CONTACT THE PERSON SUBJECT OF THE REFERRAL WITHIN THREE WORKING DAYS TO ARRANGE A FACE TO FACE APPOINTMENT TO DISCUSS THE REFERRAL FURTHER. PLEASE NOTE ADDITIONAL CHECKS WITH OTHER AGENCIES ENGAGING WITH THE PERSON MAY BE CARRIED OUT BEFORE SUPPORTED HOUSING IS OFFERED THEREFORE A DECISION MAY NOT BE IMMEDIATE.
